Permission for Transportation:  In the event where the Foundation provides transportation, I consent for my child to be transported to and/or from the above stated activities.  I designate the drivers pre-approved by the Foundation board, except those which I will state below, to provide the transportation for my child whenever taken off-premises.  The vehicle may/may not be owned by the person driving it.

Completed Code of Conduct Forms & Permission forms must be completed to participate in any event.

Forms will only be valid with an handwritten signature- no photocopies accepted. Forms submitted online will be kept on file until the date of the activity in which the form may be signed on the designated date/time prior to the activity.
